We welcome new patients who live within the practice boundary. An indication of the boundary is available in the surgery. New patients can register by collecting a registration pack from reception and you will be required to fill in a new patient questionnaire before you are seen at the practice. We require your NHS number plus a copy of your  passport or driving licence, and recent utility bill with your current address.  Acceptance of your registration will be at the discretion of the doctor. We encourage all patients to sign up to the NHS App which allows you to order your repeat medication electronically.
If this is your first GP registration since entering the UK, we will require proof of residency, and if registering a child we will require a copy of childhood vaccinations which will then be passed onto the Health Visitors in order to co-ordinate subsequent vaccinations.
There is disabled access at the surgery.
Non Residents:
if you are non resident in this country you will be asked to provide evidence that you are entitled to NHS treatment. For example an E111 or a visa. If you do not have the correct documentation you will be charged as a private patient. You will be provided with a receipt which you can use for reimbursement from your insurance company or health service in the country in which you reside permanently.
Temporary Residents:
If you are a British Citizen away from your registered GP you may be seen by a GP or nurse at the practice. You will be asked to complete a Temporary Residents form which will then be forwarded on to your usual GP.
